THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY. 

Our Commitment to Your Privacy: 

Master The Body, Inc. is dedicated to maintaining the privacy of your protected health information (PHI). We are required by law to provide you with this Notice of Privacy Practices (Notice), which explains our legal duties and privacy practices concerning your PHI. We are also required to abide by the terms of this Notice. 

How We May Use and Disclose Your Protected Health Information: 

The following categories describe the different ways we may use and disclose your PHI without your specific authorization: 

  1. Treatment: We may use or disclose your PHI to provide, coordinate, or manage your healthcare and any related services. This includes sharing your PHI with other healthcare providers involved in your care. 
  1. Payment: We may use or disclose your PHI to obtain payment for the services we provide to you. This may include contacting your health insurance company to verify coverage, billing, or coordinating with other healthcare providers for claims management. 
  1. Healthcare Operations: We may use or disclose your PHI for our healthcare operations, including quality assessment, employee review, training, licensing, and other business-related functions. 
  1. Appointment Reminders: We may use or disclose your PHI to contact you as a reminder that you have an appointment with us. 
  1. Treatment Alternatives and Health-Related Benefits: We may use or disclose your PHI to inform you of treatment alternatives or health-related benefits that may be of interest to you. 
  1. As Required by Law: We may use or disclose your PHI when we are required to do so by federal, state, or local law. 
  1. Public Health Activities: We may disclose your PHI for public health activities, including preventing or controlling disease, injury, or disability, reporting adverse events or product defects, and other related purposes. 
  1. Health Oversight Activities: We may disclose your PHI to a health oversight agency for activities authorized by law, such as audits, investigations, and inspections. 
  1. Abuse or Neglect Reporting: We may disclose your PHI to a government authority if we believe you have been a victim of abuse, neglect, or domestic violence. 
  1. Judicial and Administrative Proceedings: We may disclose your PHI in response to a court order, subpoena, or other lawful process in a judicial or administrative proceeding. 
  1. Law Enforcement Purposes: We may disclose your PHI to law enforcement officials for certain purposes, such as locating a missing person, reporting a crime, or providing information about a crime victim. 
  1. Research: We may use or disclose your PHI for research purposes when the research has been approved by an institutional review board or privacy board. 
  1. To Avert a Serious Threat to Health or Safety: We may disclose your PHI to prevent a serious threat to your health and safety or the health and safety of others. 
  1. Military and Veterans: If you are a member of the armed forces, we may disclose your PHI as required by military command authorities. 
  1. Workers' Compensation: We may disclose your PHI for workers' compensation or similar programs. 

Your Rights Regarding Your Protected Health Information: 

  1. You have the following rights concerning your PHI: 
  1. Right to Inspect and Copy: You have the right to inspect and copy your PHI that we maintain in our records. To do so, submit a written request to our Privacy Officer. We may charge a reasonable fee for copying and mailing costs. 
  1. Right to Amend: If you believe the PHI we have on record is incorrect or incomplete, you have the right to request an amendment. To request an amendment, submit a written request to our Privacy Officer, explaining why the information should be amended. We may deny your request under certain circumstances. 
  1. Right to an Accounting of Disclosures: You have the right to request an accounting of certain disclosures we have made of your PHI. To request an accounting, submit a written request to our Privacy Officer. Your request must specify a time period, not longer than six years, and may not include dates before the effective date of this Notice. 
  1. Right to Request Restrictions: You have the right to request restrictions on how we use and disclose your PHI for treatment, payment, or healthcare operations. We are not required to agree to your request, but if we do, we will abide by the agreed-upon restrictions. To request a restriction, submit a written request to our Privacy Officer. 
  1. Right to Request Confidential Communications: You have the right to request that we communicate with you about medical matters in a certain way or at a certain location. To request confidential communications, submit a written request to our Privacy Officer, specifying how or where you wish to be contacted. 
  1. Right to a Paper Copy of This Notice: You have the right to a paper copy of this Notice at any time, even if you previously agreed to receive this Notice electronically. To obtain a paper copy of this Notice, contact our Privacy Officer. 

Changes to This Notice: 

We reserve the right to change this Notice at any time. We reserve the right to make the revised or changed Notice effective for medical information we already have about you, as well as any information we receive in the future. We will post a copy of the current Notice in our facility and on our website. The Notice will contain the effective date on the first page. 

Complaints: 

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the Secretary of the U.S. Department of Health and Human Services. To file a complaint with our Privacy Officer, submit a written complaint to:

All complaints must be submitted in writing. You will not be penalized for filing a complaint.
